Before visiting the DC Office, ensure you have the following documents ready:
- Five passport-size photographs (unattested)
- CNIC authenticated photocopy of the original card.
- Original of FormB (if the applicant is below 18 years of age)
- Certified true copies of all academic certificates relevant up to the current year
- Original and Photocopy of CNIC and Domicile of the father (applicable for the applicant below or equal to 21 years of age).
- Certificate attested photocopy of service certificate in respect of any employment (for government employee)
- Utility bill – electricity or gas bill or a rent agreement.
- Photocopy of the CNIC of the spouse in case of a married person.

- Get the Form Attested: Download the Domicile application form from the DC Office, complete it with valid information, and have it attested by the Oath Commissioner.
