Initially, after the ball hit, Azhar was treated by the physio on the field. However, the batsman soon decided to return to the pavilion.
“Azhar sustained an injury to the tip of his right index finger and is currently under the care of medical staff. I will provide an update as more information becomes available,” the Pakistan Cricket Board said in a statement.
The batsman had faced only two balls and went to the pavilion without scoring any runs.
Earlier, England had announced itself. Second innings 264-7 to set Pakistan a target of 343 in the first Test in Rawalpindi on Sunday.
Harry Brooke (87), Joe Root (73) and Zach Crowley (50) all scored half-centuries for England who posted 657 runs in their first innings.
Naseem Shah, Muhammad Ali and Zahid Mehmood took two wickets each for Pakistan who took a lead of 78 runs over England.
Pakistan scored 579 runs in response to England’s first innings of 657 runs on the fourth day of the first Test.
Openers Abdullah Shafiq (114) and Imamul Haq (121) and captain Babar Azam (136) scored centuries for the home side on a flat track.
Off-spinner Will Jacks claimed 6-161 in his first Test for England.
Pakistan fast bowler Haris Rauf batted but due to right quad strain, he will not be able to bowl for the rest of the match.
England, on their first Test tour of Pakistan since 2005, will also play matches in Multan and Karachi.
